Transition Programs

What are transition programs?

Transition programs help you to make a successful adjustment from the school setting to higher education or to work. The following are all important parts of a successful transition:

  • Career exploration
  • Postsecondary education planning
  • Work-based learning
  • Independent living skills
  • Self-Advocacy and decision making
  • Connecting to adult services

What should be in a transition program?

If you are currently in high school and have an Individualized Educational Program (IEP), it should include a statement about the transition services you will need. If you become eligible for vocational services with the Division of Rehabilitation Services (DORS), you will complete an Individualized Plan for Employment (IPE).

The Individualized Plan for Employment (IPE) indicates:

  • Employment goal
  • Steps and services needed
  • Timeline
  • Service providers
  • Responsibilities
  • Criteria for success
  • Amendment process
  • Informed choice
